Digital Transformation Programme — PM&G Lead

Nottinghamshire Healthcare is one of England's largest mental health trusts, serving over 750,000 people across Nottinghamshire and beyond. In 2024, the trust was operating under CQC special measures — a designation that brings regulatory scrutiny, constrained resource, and a mandatory improvement programme running in parallel to day-to-day operations. The digital estate comprised 83 separate services using RiO and SystmOne across mental health and community care — four EPR instances in total. The transformation programme was tasked with rationalising and modernising that landscape while supporting the trust's wider recovery.

I was brought in to lead a portfolio of digital transformation workstreams spanning infrastructure, EPR strategy, clinical data quality, and service improvement:

  • Cloud migration of RiO EPR — managed the migration of two on-premises RiO instances to cloud infrastructure.
  • NHS Spine enablement — initiated the smartcard RBAC process to connect the newly migrated RiO instances to NHS Spine, enabling Care Identity Service authentication and national integration for clinical staff.
  • EPR procurement — kicked off the procurement process for replacement of both RiO and SystmOne across four instances, covering the trust's mental health and community services estate.
  • Service review across 83 services — conducted as-is process mapping across all 83 RiO-using services, identifying services recording outside the EPR (on spreadsheets), data quality gaps, and opportunities for digital enablement including AI.
  • Forms rationalisation — reviewed all referral, discharge and clinical forms with clinical colleagues, working to consolidate into a core set of standardised forms across the trust.
  • Structured data programme — identified unstructured data across services and worked with clinical teams to convert it to structured formats, improving data quality and enabling downstream analytics.

Outcomes

  • Two RiO instances migrated from on-premises to cloud
  • NHS Spine enablement and smartcard RBAC process initiated for migrated instances
  • EPR procurement process initiated for replacement of RiO and SystmOne (four instances)
  • As-is process maps completed across all 83 services; off-EPR recording and data quality gaps identified
  • Trust-wide forms rationalisation underway with clinical colleagues
  • Engagement concluded March 2026; client reference provided and verified
